Parent/Guardian Information:
Enter the contact information for the parent or legal guardian responsible for the student. Please provide an accurate phone number and email address, as this information will be used to send important event updates and the student’s scheduled appointment time.

Student Information:
Enter the information for the student who will be receiving services at the Health Fair. Please complete a separate registration form for each student who needs a sports physical.

After submitting the form, you will receive additional information by email. Your assigned appointment time will be sent in a separate email.

For sports physical appointments:

Arrive 10 minutes before your assigned appointment time.
If the student has health insurance, bring their insurance card.
Bring a completed copy of the required sports physical form with you to the appointment.
